You know this is aired on Crunchyroll back then. You should be using Erai-raws subs, instead of Horriblesubs. CR Rips subtitles of Erai-raws is untouched. While Horriblesubs is always modified to Open Sans and they change the script resolution, that's why translated sign are out position in the frame of Horriblesubs.
no, x264 is just one program you can use to encode h264/AVC video. and it's not the one 99% of companies use for BDs, so it's incorrect to label it that way.

If the Anime is aired on Crunchyroll. Mux and Use Erai-raws Crunchyroll Subs on the BD-Remux. It's unmodified. Correct translated signs positioning. Unlike Horriblesubs subtitle source it's modified to Open Sans and sometimes have the wrong translated signs positioning (out of frame.)
@Malerious10 that's interesting. I downloaded it and yeah I see that too. The JP or anime companies might just be incidentally using it from native Japanese Pegasys TMPGEnc authoring software, which includes x.264.

AVC = Advanced Video Codec
h264 = another name for AVC
x264 = the most used encoder to encode things into h264
As for why the last volume has no nipples? Who knows. They just didn't add them.
